This is a 60 minute virtual meeting (30 minute for follow-ups) which will be held via Doximity/Zoom/Phone Call. All relevant information will be sent to you prior to your appointment. NOTE (for new patients only): If you haven't already, kindly email your Insurance and Driver's License to our email ([email protected]) prior to your appointment. Also note that all copay's and deductibles will be processed before your appointment.
